Mega Session Hair Transplant: How Many Grafts in One Day, What Limits It and What Thai Clinics Publish
Part of our hair transplant guide. A "mega session" hair transplant moves a very large number of grafts in one sitting, usually for extensive hair loss. Nobody agrees where mega starts.
A 2013 review said more than 3,000 grafts in a single procedure. A webinar from the International Society of Hair Restoration Surgery (ISHRS) for surgeons said more than 4,000 follicular units in a single day.
For a transplant in Thailand, start with the ceiling your clinic publishes for itself. Absolute Hair Clinic in Bangkok says its maximum is 4,000 grafts per day. Bangkok Hair Clinic lists more than 3,000 as the maximum for shaven DHI (direct hair implantation).
Both are the clinics' own statements, and no source we found sets one safe number for everyone.
Mega Session Numbers at a Glance

What "Mega Session" Means
You'll see it spelled megasession, megasessions, mega session and mega-session. It's the same word. The number behind it isn't.

So a clinic that offers a mega session may mean 2,000 grafts or 4,000. Ask for the graft number and the hours.
This page uses roughly 3,000 grafts or more in one day, with 4,000 or more as the large end. That's our working definition, picked because the ISHRS program and Absolute's published daily maximum sit there.
What Is a Giga Session
A giga session is the same idea one step up. Merchant City Medical Group, a UK clinic group, defines it as about 5,500-12,500 grafts in a single session and says one can last two days.
A 2011 review of hair restoration surgery from Damkerng Hair Transplantation Clinic in Bangkok says mega or even giga sessions are done in selected cases. Its abstract gives no graft counts.
How Many Grafts Clinics Say They Do in One Day
Estimates by stage of hair loss are on our page on how many grafts you need. This section covers only the one-day question.
A Typical Session Is Smaller Than You Think
The ISHRS 2025 Practice Census reports an average of 2,347 grafts per session for a first procedure and 1,637 for a later one. The average case was 2,100 grafts for FUT, which removes a strip of scalp, and 2,262 for FUE, which takes grafts out one at a time.
It surveyed physician members, not patients: 992 members were invited, 946 had valid email addresses and 247 of those responded, a 26 percent response rate, with a margin of error of plus or minus 5.4 percent. Of those, 173 answered the grafts-per-session question. It covers 2024, and the report says to treat it as an estimate.
Among the surgeons who answered, 2.2 percent said their typical FUE case was 4,000 grafts or more, and 1.5 percent said so for FUT. For most of them, a 4,000-graft day isn't the typical case.
What Thai Clinics Publish
We compare what clinics publish and haven't visited them. Here's what Thai clinics print about big days.

None of the Thai clinic pages we compared states 5,000 or more grafts for a single day, and none describes a two-day plan. We read each clinic's own pages on 9 and 10 October 2026.
At Bangkok Hair Clinic the listed maximum falls as less of the donor area is shaved. Our pages on no-shave and long hair transplants cover those methods.
Absolute also publishes a donor-area photo captioned "After 4000 grafts extraction" in its donor harvesting article. It's the kind of photo worth asking any clinic for.
What a Large Session Costs
Price follows the plan. Ultima Hair Center says on its Thai-language homepage (our translation) that even for a 3,000-graft inquiry the plan has to be assessed before a cost can be given.
DHI Thailand, a Bangkok clinic that says it holds a DHI Global license, lists about 100,000-200,000 baht for 2,000-4,000 grafts, labeled 2025 data (read 10 October 2026), and says the price changes with the actual number of grafts and the area. We found no published price for 4,000-5,000 grafts at a Thai clinic and don't estimate one. For cost in context, see is a hair transplant worth it.
What Clinics Elsewhere Say
- Toronto Hair Transplant Surgeons says it regularly performs 3,000- to 4,000-graft cases in a single day.
- AlviArmani, a clinic group with clinics in the US and abroad, says it has documented a Beverly Hills case of 5,104 grafts in one day. That's a marketing claim.
- Wimpole Clinic in London usually performs around 3,000 grafts in a session and is "quite cautious about going beyond that".
- Harley Street Hair Transplant Clinic says that above 3,000 grafts the risk of over-harvesting and donor area damage rises, so it doesn't go above its range.
Asian Hair and Donor Yield
A Vancouver surgeon, Jerry Wong, wrote in a 2010 book chapter that Asian hair generally yields 3,000 to 4,000 grafts on average, with about 4,500 usually the maximum obtainable. The chapter says Wong's team tries to limit most surgeries to a maximum of 5,500 grafts.
That's one practitioner's experience, read from the chapter abstract. It isn't a rule for every Asian patient or Thai clinic.
What Limits One Session: the Donor Area
The donor area, the back and sides of the scalp where grafts are taken, is a fixed supply. The ISHRS says "Donor hair, once removed, is gone forever", and lists overharvesting the donor area among the greatest risks.
A Lifetime Budget
The International Alliance of Hair Restoration Surgeons (IAHRS) is a consumer organization that screens surgeons. In its Q&A section, an undated answer by Bernard P. Nusbaum, MD, says someone with extensive male pattern baldness has approximately 5,000 to 7,000 follicular units that can be transplanted from the donor area in a lifetime.
The answer calls that a generalization. Using most of it in one procedure leaves little in reserve as the balding pattern continues.
Wimpole Clinic says a healthy donor area holds about 6,000 harvestable grafts, and that using all of them in one session may leave no option for further transplants. Our pages on the donor area and a second hair transplant cover what's left for later.
Over-Harvesting
A 2026 review of FUE complications says overharvesting is increasingly observed in high-volume sessions exceeding 3,000-4,000 grafts, in procedures by inadequately trained teams, and in patients who ask for large counts despite insufficient donor density. The 3,000-4,000 is the review's phrase, not a measured threshold.
A 2024 paper from Chennai says mega and giga sessions pose a danger of depleting donor sites. Feller and Boucher Medical in New York says the need for more grafts makes it easy to over-harvest, which can make later transplants unlikely.
There Is No Single Safe Number
So how many grafts can a surgeon safely extract in one day? HWT Clinic in Turkey says there is "no single safe lifetime or single-session number that applies to every patient". It depends on donor density, hair caliber, scalp characteristics, extraction pattern, previous procedures, future hair loss and the size of the permanent donor zone.
Clinics and surgeons state 3,000 (Wimpole, Harley Street), more than 3,000 (Bangkok Hair Clinic), 4,000 per day (Absolute) and about 4,500 as the usual maximum yield for Asian hair (Wong). None of those is a safety study.

Every figure is a claim by its source, and they measure different things.

The spread varies because counts, methods and teams differ.
Time Out of the Body
The IAHRS answer says the longer grafts are out of the body, the greater the chance of dehydration, which it calls lethal to follicles. Long operations also raise the chance of staff fatigue during graft dissection and placement, and with it the risk of mechanical trauma to follicles.
A review in the Journal of Cutaneous and Aesthetic Surgery reports Limmer's study of grafts kept in chilled saline. Survival was 95 percent at 2 hours, 90 percent at 4, 86 at 6, 88 at 8, 79 at 24 and 54 at 48.
The numbers don't fall in a straight line, the study is older, and the review says better storage fluids may improve them. They aren't a prediction for a mega session.
Clinics describe their own handling. Absolute says it uses a first-out, first-in system and keeps grafts in a stainless bowl with a custom-made aluminum heatsink and ice gel, around 4-8 degrees for 4-6 hours without changing.
Bangkok Hair Clinic also uses first-out, first-in, and says its DHI procedure holds grafts at 2-4 degrees C in a proprietary holding solution. These are clinic claims.
The Team and Fatigue
Bernstein Medical says very large sessions increase the time grafts stay out of the body, need more surgical staff and add patient and staff fatigue, so they "should be reserved for only the most experienced surgical teams". A 2008 guideline from the dermatosurgery task force of India's dermatology association (IADVL) says hair transplantation is a team effort and large sessions need a well trained team of trained assistants.
Wong wrote that cases of 6,000 or more grafts caused shoulder, wrist and back problems for the staff who insert grafts. Feller and Boucher adds that the longer the operation, the longer local anesthetic stays in the body, which raises the risk of swelling, soreness and, in extreme instances, anesthetic toxicity.
Absolute's FUE page says most grafts are inserted with implanters by experienced assistants. Who does what on a normal day is on our page on who performs hair transplant surgery.
What Gets Riskier as Sessions Get Bigger
The One Number There Is
A multicenter study of 1,317 patients at four medical centers (Zhou and colleagues, 2024) found folliculitis, inflammation of the hair follicles, in 12.11 percent of patients after hair transplantation.
The main risk factors were surgery in summer, 4,000 or more grafts, density above 45 grafts per cm2, and a first nursing time later than 3 days. For 4,000 or more grafts the odds ratio was 4.818 (95 percent confidence interval 1.45 to 16.014), and an odds ratio above 1 means higher odds.
So 4,000 or more grafts was linked to higher odds. The interval is wide, so the estimate is imprecise, and the abstract gives no rate for the 4,000-plus group.

Skin Death and Density
The 2026 review lists megasessions above about 3,500-4,000 grafts among the technical risk factors for necrosis, meaning death of skin, at the recipient site. Dense packing and deep or overlapping incisions are on the same list. It says the true incidence is unknown.
Bernstein Medical separates the number of grafts from how tightly they're packed. It says blood flow problems track density in one area more closely than the total, so a large number over a large area doesn't create the same problems as very high density in one spot.
Swelling and Shedding
The 2026 review lists megasessions and dense frontal packing among the risk factors for swelling. Early swelling peaks around 2 to 3 days after surgery and resolves within 5 to 7 days. Our recovery timeline has the week-by-week picture.
Existing hair can shed too. Chicago Hair Institute says over-implantation into thinning hair can make some native hair fall out, peaking at six to eight weeks, and that it isn't common in properly selected patients.
The review puts shock loss at 2 to 8 weeks after surgery, with regrowth usually beginning around 3 months. It's usually temporary, but permanent loss of native hair density may occur.
The ISHRS says shock loss can occur in almost any patient. Women, and men with extensive thinning, are more susceptible, especially if hair loss hasn't been stabilized beforehand. See our page on shock loss and our overview of hair transplant risks.
FUE, FUT or Both for a Large Day
The ISHRS says FUT is known for allowing a high number of grafts in a single session but leaves a linear scar. For Norwood 6 or 7, the most advanced hair loss, it says FUT would be better in most cases, because the usable donor area is compressed and narrow. See the Norwood scale for the stages.
Thai clinics say the same. Hairsmith, BHI and HairTran in Bangkok each describe FUT as the route for a large number of grafts in one session, and none gives a number. Ultima says in Thai (our translation) that in some cases it may combine FUT and FUE for the largest possible number in one go.
Wimpole says that for 5,000 grafts it would usually combine FUT, around 3,000 to 3,500, with FUE for the remaining 1,500 to 2,000. In the ISHRS census a combined strip and FUE session averaged 2.1 percent of procedures for men.
DHI is less clear. Shapiro Medical Group, a US clinic, says DHI is less suited to this scale because its implantation step is considerably more time-consuming. Bangkok Hair Clinic lists more than 3,000 grafts for shaven DHI, and we found no study that compares the two views.

One Big Session or Two Smaller Ones
This is the choice behind most searches for a mega session. Some sources call a single big day too risky, and others favor as few sessions as possible. They're opinion more than data.
The Case for One Session
Bernstein Medical argues that the whole area that needs coverage should be transplanted in the first session, and that to maximize the total yield of hair you minimize the number of sessions. Absolute's FUT page says most patients don't want a lengthy series of treatments they might not finish.
The 2020 Guangzhou authors write that, compared with multi-stage hair transplantation, FUE megasession reduces operation frequency and overall surgery duration. Of their 273 men, 81 percent were satisfied and 19 percent had a second procedure for more density. That's one hospital's series with no control group in the abstract.
The Case for Staging
Feller and Boucher says many smaller sessions give greater control over surgery time and reduce the chance of overharvesting. Bernstein's other line is that the goal is the best result within the patient's total donor reserves, not as many grafts as possible in one session.
Wimpole says that for people with extensive hair loss or suboptimal donor density, a multi-stage approach can allow better planning and improved survival, which is a surgeon's view with no study cited.
Dr Jeff Donovan, a hair-loss dermatologist, wrote in 2015 that two 2,500-graft procedures a year apart will likely beat one 5,000-graft procedure. That's an opinion, with no data on the page. Dr Resul Yaman's clinic in Turkey says 6,000 or more grafts in a single session "is risky and may lead to an unsuccessful result".
In the ISHRS census surgeons reported an average of 1.5 procedures per patient to reach the desired result, with a median of 1. Those are surgeon averages, not patient outcomes.
Two-Day Sessions
American Mane in Miami says its mega sessions are typically two consecutive days, with 5,500-8,000 grafts. Dr Yaman's clinic describes harvesting half the donor area on day one and implanting the frontal area and midscalp, then taking the rest of the donor area and beard grafts on day two to cover the crown.
It says doctors on a two-day plan weigh overharvesting and look for a second donor source. Merchant City says a giga session can last two days, which may lead to patient fatigue.
These are clinic descriptions. No Thai clinic page we compared describes a two-day protocol, and we found no study that compares one session with staged sessions on density, donor outcome or satisfaction. If you're weighing a repeat procedure instead, see our second hair transplant page.
Who Is and Is Not a Candidate
Feller and Boucher says the patients who might benefit most have good donor density, extensive hair loss and manageable expectations. Toronto Hair Transplant Surgeons says a mega session suits larger bald areas, and that people with diffuse hair loss, or thinning hair throughout the scalp, will need other methods.
Chicago Hair Institute says most women have a diffuse hair loss pattern and risk donor depletion or shock loss, though some are acceptable candidates. That's one clinic's generalization.
The IAHRS answer says not everyone is a candidate for ultra mega sessions, since some people have tight donor areas that don't allow a wide enough strip. It also flags young patients, whose eventual baldness pattern isn't established. The ISHRS describes the ideal FUE candidate as someone whose hair is stabilized with medical therapy like finasteride and minoxidil, who is preferably older, and who has great donor hair plus reserves.
The IADVL guideline advises caution with very young patients, Norwood VI or VII patients with poor density, unrealistic expectations and significant health problems. The 2026 review adds that smoking, diabetes, hypertension and vascular disease can delay healing.

What Published Case Series Report
Published reports on large sessions are small and come from the authors' own patients. The abstracts we read describe no comparison group. They show what was done and what the authors saw, not what you should expect.
- 36 Asian men with Hamilton V-VI hair loss, a 2023 series: an average of 3,705 grafts, with patient scores for naturalness up to 4.72 and satisfaction up to 4.64 on a rating scale. The authors report that no serious side effects occurred.
- 273 men, a 2020 Guangzhou series: 3,000 to 6,000 follicular units and a graft survival of 93.5 to 96.6 percent by the authors' own count. The abstract doesn't say how survival was counted.
- 10 patients, a 2026 no-shave FUE report: an average of 4,600 extracted follicular units and a mean transection rate of 3 percent (follicles cut through during extraction). The authors say larger case-control studies are needed.
The 93.5 to 96.6 percent isn't a typical result or a promise. Our page on hair transplant success rate has the wider picture.
What Patients Describe
Patients who post about long transplant days on Reddit's hair transplant forums describe a full day at the clinic: a long stretch face down for extraction, a lunch break, then placement, mostly by assistants with the surgeon supervising.
Some say the injections hurt, and one called the hours face down the toughest part. Others pass on what their surgeon said about a one-day ceiling, and a second operation or a combined method for the rest.
Opinion on big plans splits. In a thread about a two-day plan for a very large count, replies warned about shock loss, donor depletion and having nothing left to fix mistakes with, while another commenter said their own very large procedure went smoothly.
These are forum posts from 2024 to 2026, one person's account each. They don't measure how often anything happens.
What the Published Sources Do Not Tell You
- Graft survival in sessions of 5,000 or more grafts. The undated IAHRS answer says there were no scientific studies of it, and the 2020 Guangzhou abstract covers 3,000 to 6,000 follicular units without giving survival by session size.
- Whether one session beats staged sessions on density, donor outcome or satisfaction.
- A complication rate by session size. Folliculitis in one study is the only one we found, necrosis incidence is unknown, and we found no shock-loss rate by session size.
- A safe maximum that applies to everyone.
- A Thai clinic page that advertises 5,000 or more grafts in a day, or describes a two-day plan.
- A published price for 4,000-5,000 grafts at a Thai clinic.
Questions to Ask a Clinic Before Booking a Large Session
The ISHRS training program for surgeons lists three topics for a big day: surgical logistics, team management, and techniques to maximize graft survival. Each turns into a question for you.
- How many large sessions has the surgeon performed? The IAHRS answer also suggests meeting patients who had large procedures with that surgeon and critically examining their before photographs.
- How many hairs will be transplanted, not only how many grafts? The IAHRS answer says splitting follicular units can give a large graft count without a correspondingly large number of hairs.
- How are grafts kept between harvest and placement: temperature, holding fluid, first-out first-in?
- How many hours will the day run, and where are the breaks?
- How many people place grafts, and who makes the incisions?
- What happens to the plan and the price if the day ends short of the planned count, or runs past it?
- Is a second day or a later session part of the plan, and how much donor hair is left afterward?
- What care do you give after surgery, and who do I call from home if something looks wrong?

There's no single definition. How long does a mega session take?

Is one big session riskier than two smaller ones?
The sources are opinion more than data. In one study, 4,000 or more grafts was linked to higher odds of folliculitis, and a 2026 review lists megasessions among risk factors for skin death at the recipient site. Others argue that fewer procedures protect the donor area, and we found no study comparing the two plans.
Does a mega session use up my donor hair?
It can use a large share. An IAHRS Q&A answer estimates a lifetime supply of roughly 5,000 to 7,000 follicular units in extensive baldness and warns that using most of it in one procedure leaves little in reserve. HWT Clinic says the safe number depends on donor density, hair caliber, previous procedures and future hair loss.
Who is a candidate for a mega session?
Clinics point to good donor density, extensive hair loss and realistic expectations. Diffuse thinning, tight donor areas and very young age are flagged as reasons for caution. A surgeon's exam of your donor area decides it.
Can a mega session be done over two days?
Some clinics do it. American Mane in Miami says its mega sessions are typically two consecutive days, and Dr Resul Yaman's clinic in Turkey describes harvesting and implanting in two stages. No Thai clinic page we compared describes a two-day plan, so ask any Thai clinic directly.
Do clinics in Thailand do mega sessions, and what does it cost?
