PRP After Hair Transplant: Evidence, Thai Costs, and Add-Ons
A HairTran patient paid extra for platelet-rich plasma (PRP), and afterward wrote that their surgeon said it helped healing rather than hair growth. Their recovery went smoothly, but they still wrote: "Hard to say if it was worth it or how effective it was."
PRP after hair transplant may help early healing and growth, but evidence for a better final result remains limited. We treat it as optional when planning a hair transplant, and that r/HairTransplants account explains why: a smooth recovery doesn't tell you whether the injection helped.
What PRP Is and How It's Made
What platelets are and what "platelet-rich" means
Platelets are blood fragments involved in clotting and tissue repair. PRP concentrates them in plasma, the liquid part of your own blood.
The International Society of Hair Restoration Surgery, or ISHRS, describes PRP as containing four to eight times the platelet concentration of normal plasma. That definition doesn't mean every clinic prepares an identical injection.
How a PRP session is actually prepared and given
The clinic draws blood, separates the platelet-rich portion, and injects it into the scalp or uses it during surgery. A centrifuge is the machine that spins the blood to separate its components.
The ISHRS patient page describes wide variation in collection, preparation, and administration, with no accepted standard. So ask the clinic how its preparation is made and where it will be used.
Where else PRP is used
The ISHRS also discusses PRP's history in surgery and its use in sports medicine and orthopedics. Those applications don't tell you how much additional hair a transplant patient should expect.
What PRP Is Supposed to Do After a Hair Transplant
Preserving and enhancing graft viability (PRP as a graft storage medium)
PRP can be added to the liquid holding follicles between removal and transplantation. A clinic may present this as a way to enhance graft viability during the hair transplant procedure. The ISHRS describes research suggesting improved follicle viability with this use.
A later scalp injection is a different intervention. If the clinic cites graft-storage research to sell postoperative sessions, ask how that evidence applies to its proposed treatment; our graft survival guide covers the broader surgical factors.
Promoting tissue repair and healing at the recipient site
The recipient site is the area receiving the grafts. Healing there is another proposed role for PRP, while Absolute Hair Clinic also describes using it in the donor area for healing.
Ask whether the clinic is treating the extraction sites, the implanted area, or both. A claim about easier healing needs a different explanation from a claim about more surviving grafts.
Reinvigorating dormant (non-transplanted) follicles
The ISHRS describes attempts to stimulate dormant existing follicles as experimental and needing further study. This target concerns existing hair, not transplanted hair follicles. That distinction matters when comparing hair transplants and their add-ons.
The honest bottom line: faster, not necessarily better
Earlier visible growth and greater final density are different outcomes. When a clinic says PRP improves results, ask which benefits it means and how it measures the change.
So we wouldn't pay for a promised final-density improvement on the strength of a faster-healing claim.
What the Evidence Actually Says
The 2016 randomized trial of intra-operative PRP in FUE (Garg, n=40)
Garg's 2016 study involved 40 patients having follicular unit extraction, or FUE, which removes grafts individually. It was described as a single-blind randomized study, with patients allocated alternately to PRP and non-PRP groups.
PRP was injected during surgery after the recipient slits were made. The authors reported earlier density, less transplanted-hair shedding during the hair-cycle transition, faster skin recovery, and activity in dormant follicles.
Those findings give a reason to study PRP further. A small study using PRP during surgery can't tell you what a separate course of injections will add to your final result.
The 2026 review: real evidence for thinning hair, limited for transplants
The 2026 Frontiers in Medicine review distinguishes androgenetic alopecia, meaning pattern hair loss, from transplantation. It reports density increases of about 10 to 30 hairs per square centimeter in randomized thinning-hair trials, while describing transplant evidence as limited and inconsistent between studies.
That figure isn't an estimate of extra hair after your transplant. Studies treating existing thinning hair answer a different question from studies measuring how many transplanted grafts survive.
Where PRP ranks among the add-ons
The same review ranks PRP as the most evidence-supported transplant add-on among the regenerative methods it examines. Exosome approaches remain under investigation.
That gives PRP a stronger basis for discussion than a newer label alone. It still leaves the clinic needing to explain the evidence for its own proposed use.
The ISHRS position: controlled data lacking; some investigators argue against routine use
The ISHRS explainer notes a shortage of unbiased controlled data. It also describes investigators who oppose routine PRP for transplant healing, while suggesting a possible role after earlier injury or scarring.
That is a reason for an individual medical discussion. It doesn't make prior scarring an automatic reason to buy PRP, or make every PRP offer unreasonable.
Before, During, or After the Transplant?
Ask the clinic to name the timing as well as the treatment. A quote that says only "PRP included" leaves the procedure unclear.

We wouldn't treat these timings as interchangeable. A study of injections during surgery doesn't answer whether you need repeated appointments after flying home.
How Many Sessions, and How Often?
Clinics publish different courses. These are the hair-treatment schedules described by Absolute Hair Clinic and Bangkok Hair Clinic.

Bangkok Hair Clinic uses GFC, short for Growth Factor Concentrate, to describe PRP. Neither published course supplies a universal start date after transplant surgery.
For an overseas patient, maintenance also means deciding where later sessions will happen. So ask whether the operating surgeon expects you to return or can explain the protocol to a clinician at home.
When Should You Start PRP After the Transplant?
One r/Balding poster was a month past a 2,300-graft transplant when they asked about conflicting advice. Their surgeon said to wait six months, while the online advice they had read suggested six to 12 weeks.
Neither interval is a rule for your scalp. Ask the surgeon who operated to explain the intended benefit and put the timing in writing before booking elsewhere.

Does PRP Speed Up Hair Transplant Results?
A commenter on r/HairTransplants described the hoped-for difference as seeing final-looking results sooner without necessarily getting a better result. That's the distinction to raise when a clinic sells PRP for faster growth.
Garg's small study reported earlier density and faster skin recovery with PRP given during surgery. The 2026 review still describes evidence for transplantation as limited, so no reliable number of weeks saved follows from those findings.
Compare the clinic's claim with the hair transplant recovery timeline. Ask whether it expects a visible early change or a measurable difference at the final assessment.
Does PRP Help with Shock Loss After a Hair Transplant?
Don't accept a promise that PRP will prevent postoperative shedding. The evidence described above doesn't give you a dependable prevention rate or schedule.
Patients also describe shedding after PRP itself. One r/tressless commenter reported major shedding and seborrheic dermatitis, an inflammatory scalp condition, and said they wouldn't do it again.
Another r/tressless account described increased hair loss after a second treatment, with uncertainty about the cause. If shedding changes after an injection, tell your surgeon when it started and what else changed rather than buying another session to correct it.
Does PRP Work If You Are Bald?
The 2026 Frontiers in Medicine review says PRP doesn't reverse advanced follicular miniaturization, where follicles have progressively shrunk, or replace surgery in appropriately selected patients. Don't budget for it as a way to obtain the coverage your surgeon has proposed creating with grafts.
Ask the clinic to identify which existing hairs it wants to treat. An answer about supporting thinning hair should stay separate from the transplant's coverage plan.
Safety, Complications, and Who Should Not Get PRP
Bangkok Hospital Phuket lists platelet abnormalities, sepsis, fever, infection in the treatment area, and cancers among situations where it doesn't recommend PRP. It singles out blood cancers and bone cancer.
Sepsis is a serious illness associated with infection. These exclusions need a clinician's assessment; using your own blood doesn't make a medical history unnecessary.
Ask how the clinic keeps the blood preparation and injections sterile, and what symptoms should prompt a call afterward. Agree on pain control before the session too.
One RealSelf reviewer described scalp numbing and no pain. A commenter on r/FemaleHairLoss described painful injections, particularly at the front hairline.
Is PRP FDA-Approved?
FDA stands for the US Food and Drug Administration. It regulates medical products in the United States. The ISHRS distinguishes PRP as a procedure from the device used to prepare it and says claims of FDA approval for PRP in hair transplantation are incorrect.
Its patient page explains that some preparation systems have FDA clearance for orthopedic uses, while hair-growth use is off-label. A cleared preparation device doesn't establish an approved hair-growth benefit.
For treatment in Thailand, ask which product and approval the clinic is referring to. The FDA says its oversight doesn't extend to treatments performed in other countries.
Is It Already in Your Quote, or an Extra Charge?
In a November 2023 r/HairTransplants discussion, patients described both arrangements. One said PRP was included with Dr. Laorwong, the surgeon named on Absolute Hair Clinic's PRP page, another distinguished included PRP from an extra stem-cell charge, and a HairTran patient said they paid extra for PRP.
Your written quote needs to say what applies to you. Ask whether "included" covers treatment during surgery only or later appointments as well.
A prospective patient on Bald Truth Talk asked whether dropping PRP would free up money for a few hundred more grafts. We would ask the surgeon to justify the graft plan separately, then price the add-on against that plan.
What PRP Costs in Thailand

Bangkok Hospital Phuket doesn't provide hair transplant surgery. Its package is a separate PRP service, not an add-on to surgery at that hospital.
These published hair-treatment prices aren't a standard Thai transplant surcharge. Get the operating clinic's own price for the timing and number of sessions it proposes.
Patient-reported totals that include PRP
A Bangkok Hair Clinic patient on r/HairTransplants reported a July 2026 transplant total of about US$6,650 including PRP (read 27 September 2026). That's the patient's dollar figure for the whole procedure.
An included treatment can still affect what a package costs. So ask for a quote with the optional items separated to compare the surgery itself.
Comparing a US quote and the maintenance bill
Compare your US quote with the same session count and contents in Thailand. A single injection, a multi-session course, and PRP bundled into surgery aren't equivalent purchases.
Ask for the initial course total and the proposed maintenance cost separately. Put both into the wider hair transplant cost-benefit decision before treating a low first-session price as the cost of the plan.
The Naming Confusion: PRP vs GFC vs "Stem Cell" vs "Cell Therapy"
Bangkok Hair Clinic uses GFC to describe PRP. That tells you what this clinic means; the same initials on another quote still need an explanation.
Absolute Hair Clinic lists Rigenera Activa as live cell therapy; the study discussed below spells it Regenera Activa. Ask whether a "stem cell" line means Regenera Activa. If it means something else, have the clinic name the product and say whether PRP is included.
A Bangkok transplant patient on r/HairTransplants had a stem-cell add-on alongside PRP and caffeine treatment. They wrote: "Hard to tell what's from stem cell, what's from prp and what's from caffeine solution."
Get the treatment name and contents in writing before paying.
Ask what the clinic takes from your body, what it prepares, and what it injects. A name that combines several treatments doesn't tell you which component has evidence behind it.
The Other Add-Ons Clinics Sell With a Transplant
Exosomes
Exosomes are tiny particles released by cells that carry signals between them. They are sold as another way to deliver signals involved in growth and repair.
The FDA says there are no FDA-approved exosome products. A review in Hair Transplant Forum International says the ideal delivery method, frequency, type, and concentration for hair growth and transplantation haven't been determined.
That review also identifies gaps in long-term effects and cost-effectiveness. So we wouldn't pay an exosome premium on the assumption that a newer treatment produces better graft growth.
Healthi Life in Bangkok advertises Exosome Hair Therapy at ฿90,000, about $2,720 at the conversion above (read 1 October 2026). It calls the treatment investigational.
This is a regenerative clinic's advertised hair treatment, separate from the PRP table and from transplant-package pricing. The price card doesn't tell you what an operating surgeon would charge to add exosomes to surgery.
Regenera Activa and autologous cellular micrografts
Regenera Activa uses cellular micrografts, small amounts of processed tissue from the patient. "Autologous" means the material comes from your own body.
Zari's 2021 study looked back at 140 adults treated at one center. It reported a favorable frontal-region response in 66.4%, but assessed short-term outcomes and excluded people who had undergone a hair transplant.
That result can't be used as a transplant-add-on success rate. Several r/tressless users also reported no effect, which is another reason to ask how the clinic will assess whether the treatment helped.
Mesotherapy and laser
For mesotherapy, ask what substance is being injected before comparing it with PRP. A patient on r/HairTransplants couldn't tell whether their postoperative mesotherapy helped because new transplant hairs were already appearing.
Low-level laser therapy, often shortened to LLLT, uses light as a hair treatment. The 2026 Frontiers in Medicine review describes limited evidence for laser after transplantation and emerging combinations, so a laser cap shouldn't become an assumed requirement in your surgery budget.
What Patients Actually Say
One RealSelf reviewer reported no major improvement after three PRP sessions. Another RealSelf reviewer reported perceived new growth at a three-month follow-up.
At the skeptical end, a commenter on r/HairTransplants called PRP "snake oil." The small positive trial gives more reason for a measured discussion than that dismissal does, but the mixed experiences don't provide a dependable extra-growth estimate either.
The HairTran patient's smooth recovery and uncertainty can both be true. Having surgery and several add-ons together makes it hard to identify which treatment changed what.
So ask for a record of each treatment given during the operation and each later session. At follow-up, ask the clinic to assess the outcome it originally said PRP would change.
How to Decide: Questions to Ask Before You Pay
We would consider PRP when the surgeon explains its role, the full cost is clear, and a possible early benefit is worth that cost to you. We would decline a bundle whose contents are unclear or whose sale depends on a promise of better final density.
Ask the clinic:
- What is being offered: PRP, GFC, exosomes, Regenera Activa, or more than one treatment?
- Is it included, and what would the transplant quote be without optional add-ons?
- What is the total for the initial course and proposed maintenance?
- When will it be given, and what is it meant to change?
- What evidence supports that particular use and outcome?
- What screening will you do before deciding whether I should have it?
If you're comparing clinics, request an itemized quote and ask for optional treatments on separate lines. Get the surgeon's explanation before committing to the course.

It can be reasonable if the surgeon explains a defined use and you're comfortable paying for an uncertain extra benefit.
