Will They Charge Me More If the Graft Count Ends Up Different?
One Bangkok Hair Clinic patient was initially quoted 2,800 to 3,300 grafts, then reported a final count of 2,725. If you're planning a hair transplant in Thailand, the useful detail is the written pricing formula that patient received before booking.
A higher final graft count can mean a higher bill, and fewer grafts don't automatically mean a refund.
The r/HairTransplants poster described a fixed charge for the first block of grafts, then a rate for each extra graft. Your own written terms need to explain both directions before you commit.
The Short Answer: It Depends on the Pricing Model
A quote needs to say how the clinic turns a graft count into a bill. A price beside an estimated count doesn't answer what happens outside that estimate.
| Pricing model | How the total is calculated | What to clarify if the count changes |
|---|---|---|
| Per graft | The billed count multiplied by the applicable rate, subject to any minimum | Whether the rate stays the same and which count gets billed |
| Fixed block plus per graft | A charge covering an initial block, then a rate for additional grafts | Which grafts the block covers and the charge above it |
| Graft-count tiers | A package price tied to a count or band | Whether a different count moves you into another tier |
| Flat package | One price under the package's agreed conditions | The graft allowance and the terms for fewer or extra grafts |
Hair Clinique by SLC describes actual-count billing as paying for grafts used, while a flat package depends on its conditions. Translated from Thai.
We would get the rule for a changed count alongside the initial total. For a package, that means asking whether the quoted price covers the full assessed range and what happens beyond it.
Run the Quote-Range Stress Test Before You Pay a Deposit
Take the low and high ends of the doctor's estimate and ask the clinic to price both. Then ask for the rule outside that range and any maximum total it will agree to in writing.
The Bangkok Hair Clinic patient provides a worked example. In the July 2026 r/HairTransplants account, the poster wrote: *"129,000THB for first 1500 grafts and additional 75THB per graft."*
| Part of the quote | What this patient reported or what the formula gives |
|---|---|
| Estimated graft range | 2,800 to 3,300 grafts |
| Pricing model | 129,000 THB for the first 1,500 grafts, then 75 THB per additional graft |
| Low-end estimate | 129,000 + (2,800 - 1,500) x 75 = 226,500 THB |
| High-end estimate | 129,000 + (3,300 - 1,500) x 75 = 264,000 THB |
| Reported final count | 2,725 grafts |
The calculations are our arithmetic using the patient's reported terms. They match the estimate range the patient described.
The high-end estimate is what the formula produces at 3,300 grafts. Calling it a maximum would require a separate agreement that the bill cannot exceed it.
For your own quotes, keep included care and excluded charges beside these totals. If the clinic gives you a package price, ask it to confirm whether both ends of your assessed range fit that package.
Ask for the maximum payable separately from the estimated total.
The clinic might leave the final price dependent on the count. In that case, get the extra-graft rate and the approval process in writing so you can decide whether that arrangement fits your budget.
How Hair Transplant Pricing Actually Works
Per-Graft Pricing
With a per-graft quote, the count and the rate both matter. A lower count can reduce the calculation, but a minimum charge can limit how far the total falls.
HairTran says its price may adjust to the actual graft count needed, assessed from the thinning or bald area and donor-hair density on procedure day. Translated from Thai; read 22 October 2026.
That statement makes the timing relevant to your budget. Ask whether the clinic will confirm the revised count and total before proceeding, and how it will handle any later change.
A revision clause should explain what can change. So get separate answers for the graft count, the rate charged for those grafts and any additional services.
Fixed / All-Inclusive Packages
A fixed package needs a defined graft allowance. The word "all-inclusive" also needs an inclusion list before you can compare it with a per-graft quote.
Ask whether the package covers the doctor's whole estimated range. If the final plan falls outside it, ask whether the clinic changes the package, adds a charge or leaves the price unchanged.
Keep those questions separate from medication and aftercare. An answer about included services doesn't tell you how the clinic bills a changed graft count.
What Happens When the Count Is Higher Than Quoted
Under Per-Graft Pricing, Extra Grafts Can Raise the Bill
If your agreement charges by the actual count at the same rate, extra grafts increase the total. In a block-plus-per-graft quote, apply the additional rate only to grafts above the included block.
One patient reporting treatment with Dr. Ratchathorn described an estimate of about 3,500 grafts and a final count of 4,624. The September 2026 r/Hairtransplant post explains how the increase was discussed:
> During my graft harvesting they said my donor was much stronger than they anticipated and asked if I wanted to receive more grafts which I agreed to.
That patient agreed to the extra grafts during harvesting. Before your own procedure, ask how the clinic would explain the additional charge and record your agreement if it proposes more grafts.
A count increase and a price increase need separate answers. Ask what the new total would be before agreeing to the extra work.
Under a Flat Package, Check the Graft Limit
Extra grafts may fit within the package you've agreed to, or they may fall outside its terms. Ask for the boundary and the price consequence in writing.
The package price also needs an agreed treatment plan. Ask which areas the surgeon intends to treat within the assessed count, rather than using the package's advertised graft allowance to decide the plan yourself.
What Happens When the Count Is Lower Than Quoted
Actual-Count Billing Can Mean a Lower Total
The Bangkok Hair Clinic patient reported 2,725 grafts after an initial estimate of 2,800 to 3,300. The account's pricing formula gives you a way to check how a lower count affects the graft charge.
Under that formula, the initial block still costs the same. Only the number of additional grafts changes, so dividing the entire quote by the estimated count would give you the wrong rate to subtract.
Ask how any money already paid would be reconciled with the final bill. Get the answer before booking rather than assuming a lower count creates an automatic refund.
Under a Flat Package You May Still Pay the Full Price
Absolute Hair Clinic describes a flat package as one price regardless of the grafts needed. Its example warns that someone receiving far fewer grafts than the package describes can still pay the package price (translated from Thai; read 22 October 2026).
Ask what happens if the final assessment puts you below the quoted range. The answer should specify whether the clinic changes the package price or keeps the original total.
If the lower count changes the planned coverage, ask the surgeon to explain that change too. The billing question and the treatment-plan question both need answers.
Grafts vs Hairs: What You Are Actually Paying For
HairTran says one graft contains about one to four hairs, depending on the roots at each point. Translated from Thai.
So a hair count and a graft count aren't interchangeable. Hairsmith says some clinics count individual hairs while others count grafts, and advises patients to clarify the unit being charged.
Ask the clinic to use that same unit in the estimate and final bill. If it supplies both totals, ask it to label them clearly so you can compare the final graft count with the quoted graft count.
How the Estimate Is Made and Why It Moves
What the Doctor Assesses Before Confirming the Count
HairTran says a doctor assesses the graft count because scalp structure and donor density differ between people. Translated from Thai.
The donor area is where the hair for transplantation comes from. The size of the area being treated and the available donor hair therefore belong in the discussion of your proposed count.
Keep the initial estimate, the agreed plan and the final recorded count clear in the clinic's paperwork. Ask when each is confirmed and how the clinic records the number transplanted.
An assessment explains why the planned number may change. You still need the clinic to explain how its final recorded number becomes the billed count.
Why a Photo Estimate Can Fall After Hairline Design
A HairTran patient described a photo estimate of 2,500 grafts that became 1,800 after the hairline was drawn. The October 2024 r/HairTransplants poster wrote:
> From seeing a photo she calculated 2500 Grafts but after she created the hairline it came down to 1800 Grafts.
The same patient said other photo assessments had reached 3,000 grafts. When your quotes differ, ask each doctor what area and hairline the estimate covers before comparing the prices.
A lower count alone doesn't explain whether two doctors are proposing the same work. Get the proposed coverage beside the graft range so the difference is something the surgeon can explain.
Red Flags in a Graft-Count Quote
An Estimate Needs an Extra-Graft Rule
An estimate can change, so ask for the calculation that will apply if it does. A quote that gives a count without explaining the price outside that count leaves your possible bill unresolved.
Hair Clinique by SLC identifies "flat rate = unlimited grafts" as a misunderstanding and advises checking how many grafts the flat price covers. Translated from Thai.
So ask what happens below the range as well as above it. A clear extra-graft rate answers only half of your billing question.
Donor Capacity: Why the Count Has a Ceiling
Bangkok Hair Clinic says available donor grafts and the size of the treatment area must be considered. It describes staged treatment or placing density in priority areas for advanced cases (translated from Thai).
Ask the surgeon what would change if the proposed count cannot be supplied in that session. Then ask the clinic how that revised plan would affect the agreed price.
How Will the Clinic Record Your Final Graft Count?
Ask what record you will receive of the final graft count and how it will relate to your bill. Have the clinic explain what it counts, who records it and whether the number refers to grafts transplanted.
If you're charged by the graft, you need enough detail to reconcile the count with the agreed rate or block formula. For a package, ask how the recorded count relates to the graft allowance you agreed to.
A written total gives you something to compare with the bill. Ask separately whether the clinic offers any way to check that total; don't assume a certificate, video or independent count is available.
Thailand in Practice: How Bangkok Clinics Actually Price Grafts
We compare what clinics publish; nobody at AsiaPatient has visited these clinics or had a procedure at them. The clinic-published prices below were read 22 October 2026; the Bangkok Hair Clinic row is a patient's July 2026 account.
| Clinic or patient account | Stated pricing | What this means when comparing your quote |
|---|---|---|
| Hairsmith | 80 THB per graft, labeled 2025 pricing | Get the applicable minimum and final billed count confirmed |
| HE Clinic | Shaven implanter-pen packages: 99,000 THB for 1,000 grafts; 119,000 THB for 2,000; 149,000 THB for a package over 2,800 | Ask which tier covers your assessed range and what happens outside it |
| Bangkok Hair Clinic patient on r/HairTransplants | Reported 129,000 THB for the first 1,500 grafts, then 75 THB per additional graft | The additional-graft rate applies only above the block; this is an individual account |
These prices use different calculations. Multiplying every graft by the BHC patient's additional-graft rate would leave out part of the reported charge.
A package also needs its technique beside the count. HE's row describes shaven implanter-pen work, so don't use it as a price for a different shaving plan.
Two quotes with different graft counts need an explanation of the planned area and coverage before you can compare their totals. Even similar counts need the same technique and inclusion checks; the count alone doesn't make the quotes equivalent.
For other published prices, see our Thailand clinic cost comparison. So keep your own assessed graft range beside any rate you use.
What Patients Actually Report (Thailand)
Zack, writing on Hair To Freedom, described Absolute as upfront about pricing. Zack also wrote, *"The additional post-operative care I paid for was my choice rather than something sprung on me afterwards."*
When discussing the final total, ask the clinic to separate any graft-count adjustment from optional care you've selected. That lets you see which part of the price changed and why.
How to Lock the Price Before You Book
The Questions to Ask (and Get in Writing)
B-suke reported receiving an estimate through LINE and wrote, *"There was no formal quote document."* The B-suke account is a reason to ask for one complete fee summary before relying on an estimated total.
We would ask the clinic to answer these questions together:
- What graft range has the doctor assessed, and when will the planned count be confirmed?
- Is the price per graft, a fixed block plus extra grafts, a graft-count tier or a flat package?
- What happens to the price if the final count is above or below the range?
- What is included, and which services can cost extra?
- Can you state a maximum total payable for this planned procedure?
- How will the final transplanted graft count be recorded and matched to the bill?
A maximum is something to request. The clinic pages cited here don't publish a maximum-payable policy for a changed graft count.

What a Written Fee Summary Should Contain
The BHC patient's summary paired an estimated range with a formula. Ask your clinic to do the same, then explain what happens outside that range.
You can send this request:
> Please confirm my assessed graft range and the total price at both ends, including the listed care. Please explain the charges if the final count is higher or lower, and whether you can agree to a maximum total. > > If additional grafts are proposed during the procedure, please explain how you would confirm the extra cost with me. Please also tell me how the final graft count will be recorded and shown against the bill.
If you want help comparing written Thailand quotes, contact AsiaPatient with the proposed counts and pricing terms. Ask the clinic to resolve any unanswered price term before paying a deposit.
Frequently asked questions
If the final graft count is different from the quote, will I have to pay more at the end?
A higher count can raise an actual-count bill. A package may cover the change or have a boundary that affects the price, so get the applicable rule in writing before booking. Ask for the total at both ends of your assessed range and whether the clinic will agree to a maximum payable.
What if the clinic transplants fewer grafts than quoted?
Ask whether your agreement recalculates the bill using the final count. A fixed block or package can retain its charge even when fewer grafts are used. Get any lower-count adjustment and the treatment of money already paid written into your fee summary.
How many grafts were actually used?
Ask the clinic for the final recorded graft count and an explanation of how it relates to your bill. A reader asked this question in a HairTran patient thread on r/HairTransplants. Make sure the answer gives grafts rather than individual hairs, and ask how the clinic documents that number.
Why do Bangkok clinic quotes vary so much?
Compare the billing model and applicable rate, then the assessed count, technique and inclusions. One patient comparing Bangkok quotes wrote, *"The cost variation is huge,"* on r/HairTransplants. Ask both clinics to explain the proposed work before treating different totals as prices for the same procedure.
What if I only have an estimate in a chat message?
Ask for a complete written fee summary that includes the graft range, pricing formula, inclusions and rules for a changed count. B-suke's LINE account describes an estimate without a formal quote. A number in a chat doesn't answer those questions unless the accompanying terms do.
