Hair Transplant Scabs: When They Fall Off and How Thai Clinics Say to Wash Them
In December 2024 a poster on r/Hairtransplant asked when to start removing scabs, eight days after surgery. One commenter wrote "My doctor gave instructions to massage them out on Day 10, but the consensus seems to be in the 7-14 day range?" and another "My doctor advised ON DAY 14". (u/GorillaKingSound and u/benno5555, 6 Dec 2024)
Hair transplant scabs are a normal part of healing, and scabs generally last about one to two weeks in the sources below. The clinics quoted here disagree on when to start washing and rubbing them off, but each one that addresses picking says not to. This page is part of our hair transplant guide.
Hair Transplant Scabs at a Glance
- Usual range. The ISHRS (International Society of Hair Restoration Surgery) says tiny scabs stay "until they are gently rubbed away usually 1 to 2 weeks later".
- Picking. In a 42-patient study, "Pulling on an adherent scab always resulted in a lost graft through day 5." (Bernstein and Rassman, 2006)
- The 14-day line. Absolute Hair Clinic says "Scabs still firmly attached after Day 14 = consult your doctor." A 2026 review says persistent crusts beyond 2 weeks "should therefore be evaluated".
- When to call. Yanhee International Hospital lists bleeding that won't stop with gentle pressure, spreading redness or pus, fever, and pain that gets worse after the first few days.
What Are Hair Transplant Scabs, and Why Do They Form?
The NHS lists "temporary scabbing where the hair was transplanted" among the effects it calls common after hair transplants. Cleveland Clinic lists "Crust or scabs" among the side effects of hair transplant surgery that it calls usually temporary.
A 2026 review of FUE (follicular unit excision) complications in Frontiers in Medicine calls scabbing part of normal healing: "Crust formation is a normal part of postoperative healing after FUE, typically appearing within 2-3 days and resolving spontaneously by 7-10 days with appropriate care."
Where Scabs Appear
Scabs form wherever the skin was cut: the recipient area, where the grafts go in, and the donor area, where the hairs are taken from.
Hairsmith Clinic says blood stains on the recipient area "start to harden up and become scabs" on the second day. Yanhee International Hospital describes the FUE donor area as "a field of small round dots that scab over and are rubbed away at one to two weeks".
Scabs typically appear within 2-3 days, according to the review. HairTran's Thai article says they begin to form clearly in the first 1-3 days and start to harden around days 4-7 (our translation).
Why the Body Makes Scabs
HairTran Clinic's Thai article explains it (our translation from Thai): the skin releases lymph fluid, blood and platelets to cover the wound, and when these fluids meet the air they dry and become scabs. So scabs are made of dried blood and wound fluid sitting over each tiny wound.
Are Scabs Dangerous?
HairTran says scabs are not dangerous but become a problem if they get too thick or stay too long (our translation). The 2026 review gives the medical reason: "Persistent or thick scabs can delay re-epithelialization and sustain local inflammation, potentially compromising graft growth". Re-epithelialization means the skin growing back over the wound.
When Do Scabs Fall Off After a Hair Transplant?
No source gives one fixed day. The table shows what each one says.

Most of the ranges sit between day 7 and day 14. Thai Hair Clinic's 2-4 weeks is the main exception, and Yanhee puts the donor-area crusts at days 5 to 7.
The day varies. So follow the day on your own clinic's instructions.
Why the Day Ranges Differ
- HairTran's Thai article says scabs in places with less blood supply or a drier wound, such as the front hairline, can come off slightly later than in other spots, which is normal (our translation).
- The ISHRS adds that "different surgeons have different policies".
- Bangkok Hair Clinic's Thai article says most scabs gradually come off by themselves during hair washing within the first 2 weeks (our translation), and the clinics start washing on different days, as the tables below show.
Hairs That Come Away With the Scabs
Transplanted hairs shed in the weeks after surgery, and some leave with a scab during healing. The AAD says: "It's important to know that between two and eight weeks after the surgery, the transplanted hair will fall out. This is normal."
Absolute Hair Clinic says "The shaft of the hair will shed with these scabs. This is normal" and that "The roots remain behind", which is the clinic's claim. Thai Hair Clinic and Bangkok Hair Clinic also describe hairs leaving with scabs as normal shedding, and HairTran says it's usually hair shedding in its normal cycle if there is no heavy bleeding (our translation).

Why You Should Not Pick or Scratch Scabs
Picking is the rule the clinics quoted here share. The reason is how firmly a graft is held in the skin, which one study measured.
What the Graft-Anchoring Study Measured
In a 2006 study in Dermatologic Surgery, Bernstein and Rassman had several grafts pulled by hand in each of 42 patients to find when they could no longer be dislodged. They reported:
- "Pulling on an adherent scab always resulted in a lost graft through day 5."
- "By the sixth day, pulling on a hair no longer dislodged the graft."
- "At 9 days postoperatively, grafts were no longer at risk of being dislodged."
- "The presence of crusting extends the interval that grafts are at risk of being dislodged postoperatively."
That's one study of grafts pulled by hand, and its abstract doesn't name the technique. So it can't tell you which day is safe for you.
A 2015 column by Sara Wasserbauer, MD, in the ISHRS member journal says grafts are "very vulnerable" through day 5. It lists the 2006 study as its first reference, so it isn't a second study.
What Clinics Say About Picking
Every clinic in the table says not to pick or scratch. The periods they name run from the first 7 days to the first 2 weeks.

The NHS says to be "very careful with your transplanted hair" for the first 2 weeks, because the grafts aren't secure. HairTran adds, in its Thai FAQ, that rubbing the scabs too hard yourself may make them bleed and turn into a sore (our translation).
If a Scab Comes Off and It Bleeds
HairTran's Thai article says that if you pick a scab by accident and it bleeds, dab with clean gauze for about 5-10 minutes and tell the doctor right away so they can assess whether a graft came out too (our translation). Yanhee says to call for "bleeding that does not stop with gentle pressure" or "if a graft comes out in the first days", while Thai Hair Clinic calls "slight bleeding on the donor or grafted area" normal.
HairTran's Thai washing guide adds that if only 1-2 grafts come away with no bleeding, they may be only scab tissue, while one that bleeds means the root has really come out (our translation).
Bernstein Medical, a New York clinic included for comparison, answered a patient who asked about day 5 that the grafts at five days "are pretty secure, but still can be dislodged", and that with no bleeding "it is unlikely that you lost any grafts" (its answer is dated 5 April 2007).

How to Remove Scabs After a Hair Transplant: What Thai Clinics Teach
This section sets out what each clinic's own website says about washing and removing scabs after hair transplants. We compare what the clinics publish. We haven't visited them, and nobody at AsiaPatient has had the procedure.

The First Days: When Washing Starts and How

So the first wash runs from the day after surgery with a clinic wash (HairTran, Thai Hair Clinic), to 24-48 hours (Absolute Hair Clinic), to a 7-day wait on Cosmo Clinic's English page. Yanhee says its day-1 visit "is not skippable for anyone".
Softening and Rubbing Off Scabs
The clinics that teach a softening step describe it in similar terms: oil or baby shampoo left on, then gentle washing with the fingertips. The start day, soak time and products differ.

Absolute Hair Clinic and HairTran say fingertips, never nails. Hairsmith says fingertips, and Bangkok Hair Clinic says the pads of the fingers. Soak times run from 5-10 minutes to 20-30 minutes, with an overnight option on one clinic's page.
If Scabs Remain: What the Clinics Say They Do
These are each clinic's statements about its own service. They aren't independent facts or a price, and whether any of it applies to your booking is a question for the clinic.
- HairTran Clinic says that after the full 14 days the doctor books a follow-up to wash the hair and soak with olive oil to remove the remaining scabs, at no additional charge (our translation from its Thai article).
- Hairsmith Clinic lists "Free post-transplant shampoo and scab removal" among what its cost page says is included.
- Absolute Hair Clinic says it provides five days of post-operative care that includes wound care and a hair wash.
- Bangkok Hair Clinic says that for leftover scabs you can contact the clinic so the medical team can assess and clean it safely (our translation).
Where One Clinic's Own Pages Disagree
Several clinics give different numbers on different pages. Absolute Hair Clinic's scab page says to start gently rubbing while shampooing after day 7, while its aftercare page puts soaking and rubbing scabs off after day 10. HairTran's Thai article starts massage on day 10, while its English page says that "After 15 days" you can apply olive oil to soften the scabs and rub them off.
Hairsmith gives 5-10 minutes on one page and 10-15 on another for the same soak. Its scab page has scabs starting to form on the second day, while its timeline page says they form on days 4-7.
Cosmo Clinic's English page says no washing for 7 days. Cosmoprime Clinic's 2022 Thai page says about 1 week for the front scabbed area, and its 2025 Thai page says the front area after 2 weeks. Both sites use the Cosmoprime Clinic name.

Wash Early or Wait? What the Evidence Shows
What Clinics and Health Bodies State
Outside Thailand, the sources lean earlier:
- Bernstein Medical in New York said in an answer dated 17 November 2016 that "The purpose of showering the day after the procedure is to remove scabs and dried blood", for "quicker healing, less inflammation (redness), and a reduced incidence of infection". That's a US clinic describing its own protocol, not a trial result.
- The 2026 review says "Early gentle cleansing with saline or mild shampoo within 24-72 h promotes faster re-epithelialization and reduces bacterial colonization".
- Cleveland Clinic lists "Day 2: Wash your hair." and says recovery depends on the type of transplant.
- The NHS says "Day 6: you should be able to wash your hair gently by hand."
Cosmo Clinic's English page says to wait 7 days. HairTran's Thai washing guide warns that skipping washes out of fear of losing grafts can backfire: scabs may thicken and the follicle openings may become blocked (our translation).
What the Studies Show
Bernstein and Rassman's authors write that preventing crusts "would both shorten the time patients are at risk of losing their grafts and enable them to return to their normal hair care routines more quickly". Their abstract doesn't test a way of preventing crusts.
The 2026 review says delayed scab detachment "may increase the risk of perifollicular inflammation or folliculitis, often related to inadequate washing or premature manipulation", meaning inflammation around the follicles. It also describes a study where folliculitis was more likely with "delayed postoperative washing (>3 days)", listed alongside very large sessions, dense implanting and summer procedures.
A 2026 pilot study put 21 men on "a structured 10-day washing protocol starting 48-72 h post-surgery" with a salicylic acid shampoo. It had no comparison group and was financially supported by Cantabria Labs, so it doesn't show that one protocol beats another. Its authors say postoperative scalp care "remains poorly standardized".
Is a Scab Protection or Something to Prevent?
Bangkok Hair Clinic's Thai article calls the scab a natural bandage that protects the roots underneath while they are not yet firmly attached to the surrounding tissue, and a good sign that the wound is healing well (our translation).
The 2015 ISHRS column takes the other side: "Try to prevent the crusts! Grafts don't need them to survive and they may in fact inhibit their survival." That's one surgeon's educational answer, not a study.
What No Source Settles
None of the studies and reviews quoted on this page compares two washing schedules head to head. On aftercare policies in general, the ISHRS says they "vary among surgeons, so it is important to follow the instructions given by the surgeon you choose rather than any information found on other websites".
The decision rule: follow the written instructions from the clinic that operates on you, and ask the clinic to explain any step you don't understand. Use the days on this page to ask better questions, not to override those instructions.
What Patients Say on Reddit About Hair Transplant Scabs
These accounts come from four Reddit threads. Each is one person's experience, shown with the poster's handle and the comment date (UTC, as Reddit's feed gives it), and none is medical advice or verified.
Clinics Gave Different Days
The r/Hairtransplant thread "8 days post op - when should I start removing scabs?" drew these replies:

Start days in that thread run from day 8 to day 14, and soak times from about half an hour to three hours.
Hairs in the Scabs
In the thread "Scabs with Follicles? Lost grafts or overthinking post washes? Day 10 vs Day 12", the original poster, u/Motor-Resource3381, wrote on 23 Feb 2025 that 85% of the scabs came off after a light fingertip massage on day 10, their own estimate. The next day they relayed their clinic's post-op reply, which said hairs coming away with scabs may happen and "is not permanent hair loss". That's a clinic reply passed on by a patient, not a promise.
Another commenter, u/Wolfie4836 (23 Feb 2025), wrote "Some were just hairs others were the hair/scab combo." and "My scabs were pretty much gone by days 12-14."
Patients Who Say They Had It Done in Bangkok
Besides u/terpzldn in the table above, u/bandana_bro_01 wrote up their own Bangkok transplant in r/HairTransplants on 13 Oct 2025. On days 10 to 16 they saw no isolated scabs but a crust across the whole implanted area, and after they started rubbing on day 12 the "skin went from having the whitish layer due to the ointment to super red (which scared me a lot)".
Why Reddit Is Not Instruction
The advice conflicts. In the thread "Scabs or grafts?", u/strictly4him (9 Sep 2024) wrote "Use your thumb or a cotton wool to clean them out." while u/weddingchimp5000 (10 Sep 2024) wrote "Yea try some oil. Can be baby oil too. That's what my clinic used to remove the scabs". The clinics above say fingertips or the pads of the fingers.
Some replies state things flatly. u/Rocko210 (15 Dec 2025) wrote "No. The big ones need to be removed by hand. The small tiny ones will fall off naturally." The clinics on this page say to soften scabs and let them come away during washing.
The commenters keep ending in the same place. u/SonicB0000M (6 Dec 2024): "You won't get true answers here, as every clinic has different guidelines lol". u/PossessionJust5723 (6 Dec 2024): "Always best to follow the advice of your doctor."
When a Scab Is a Reason to Contact Your Clinic
A scab on its own is part of healing. These are the symptoms and situations where the sources say to get in touch.
Scabs Still Attached After About Two Weeks
- Absolute Hair Clinic: "Scabs still firmly attached after Day 14 = consult your doctor."
- HairTran says to consult the doctor or go back to the clinic if scabs haven't all come off after 14 days (our translation), and Bangkok Hair Clinic says to contact the clinic in week 3 (our translation).
- The 2026 review: "Persistent crusts beyond 2 weeks should therefore be evaluated, as they may indicate delayed healing."
Signs That Call for the Clinic
- Absolute Hair Clinic says redness, pus or swelling "may indicate infection".
- Yanhee International Hospital says to call for bleeding that doesn't stop with gentle pressure, spreading redness or pus at the donor or recipient site, fever, pain that "gets worse rather than better after the first few days", or a graft coming out in the first days.
- The NHS says to contact the clinic "as soon as possible if you have severe pain or any unexpected symptoms". Its fever page says "A high temperature is usually considered to be 38C or above."
- Cleveland Clinic lists a fever that doesn't go away, excessive bleeding from the scalp, severe redness or pain, and signs of infection such as green or yellow pus.
The 2026 review says "Typical clinical signs include pustules or honey-colored crusts, increasing pain, swelling, erythema, purulent drainage, and tenderness or enlargement of regional lymph nodes." Erythema is redness and purulent drainage is pus.
It also calls infection after FUE rare, "with most studies reporting an incidence below 1%, although published rates vary across series". That figure is for infection, not for scabbing. Signs and rates are covered on our hair transplant infection page, and other complications on our risks page.
Thick Crusts Early, With Oozing
The review says "the early appearance of thick crusts accompanied by increased exudate should raise suspicion of ischemic injury or evolving recipient-site necrosis". Exudate is fluid oozing from the wound, and ischemic means reduced blood supply. That's a clinical call, so thick, oozing crusts in the first days are worth showing to your clinic.
If You Fly Home Before the Scabs Are Gone
Scab timelines of 10-14 days, and Thai Hair Clinic's 2-4 weeks, can outlast a short trip. HairTran's English FUE page says to "stay in Bangkok for at least 3-5 days" and that follow-ups "can be done online" for clients living overseas. Ask what an online follow-up covers for scabs and how quickly photos are answered.
Yanhee says how long to stay "is the question to settle before you book flights". Its stay table gives 48 hours as the minimum for shaven FUE, for the day-1 review, dressing removal and first professional wash, and about one week as a comfortable stay. Thai Hair Clinic says to return to the clinic for a hair wash every day until your departure date.
Ask your clinic three things in writing: who answers messages after you leave, what photos or video they need, and what they treat as urgent.
Questions to Ask Your Clinic About Hair Transplant Scabs Before You Book
- Who does the first wash, and on which day? The clinics above range from a clinic wash the day after surgery to a 7-day wait.
- Which shampoo and other products will the clinic give me, and do I tap them on, pat them or rub them?
- On which day may I start rubbing, and with what: fingertips or a cotton bud?
- What do I soak scabs in, and for how long? The pages above give anything from 5-10 minutes to overnight.
- Does the aftercare change with the procedure I book?
- Is a scab-removal wash included in my plan, on which day, and what does it cost?
- If I leave Thailand early, who checks my scalp, and how do photos or video calls work?
- Who answers messages after I leave, and how fast?
- What counts as normal healing, what do you treat as urgent, and which number do I use?
- What should I do if a scab comes off early and bleeds?
- Can I have the aftercare instructions in writing, in English?

Clinics name baby shampoo, or baby, olive, rosemary or coconut oil, left on for 10-15 minutes (Absolute Hair Clinic), 15-20 minutes (Bangkok Hair Clinic) or 20-30 minutes (HairTran). Hairsmith says 5-10 or 10-15 minutes. Then wash gently with the fingertips, not the nails.
What if scabs are still there after two weeks?
Absolute Hair Clinic says to consult your doctor if scabs are still firmly attached after day 14, and HairTran says to consult the doctor or return to the clinic if they haven't all come off. Bangkok Hair Clinic says to contact the clinic in week 3 and not to pick them off yourself. A 2026 review says persistent crusts beyond 2 weeks should be evaluated.
Do scabs mean I have an infection?
Not on their own, since the NHS lists temporary scabbing among the effects it calls common after hair transplants. A 2026 review lists pustules or honey-colored crusts, increasing pain, swelling, redness and pus as signs of infection, and calls infection after FUE rare. Call your clinic about spreading redness, pus, fever or pain that gets worse.
Do Thai clinics remove leftover scabs for you?
Some say so on their own pages. HairTran says it books a follow-up at 14 days to wash and soak with olive oil at no additional charge, and Hairsmith's cost page lists "Free post-transplant shampoo and scab removal". Thai Hair Clinic says to return for a wash every day until departure, so ask what your own booking includes.
