Crown Hair Transplant Before and After: What the Crown Actually Looks Like at 6, 12 and 18 Months
In December 2023, a patient who had a 3,300-graft DHI/FUE procedure at HairTran in Bangkok posted a six-month update on r/HairTransplants. The bald spot on their crown was still there. It had shrunk and filled in somewhat.
Our hair transplant guide covers the full decision. On the crown, that six-month picture answers the first part of the question: the crown can still look thin at six months.
Surgeons generally ask you to wait 12 to 18 months before judging the final result. HairTran schedules its own final assessment at 12 months, with the doctor comparing before-and-after photos.
Another r/HairTransplants poster described the search problem in January 2024: "All I see is photos from the front I just want to see how it looks on the crown after as that's where my balding it the worst."
This page sticks to the crown's before-and-after timeline and how to judge a crown photo. For technique and graft planning, see our crown hair transplant guide.
Why the Crown Is the Slowest Area to Fill
The International Society of Hair Restoration Surgery (ISHRS) says that if the crown is the area you want transplanted but the surgeon treats only the hairline, "that would not be a good situation." So a hairline result says little about the crown, unless the crown was treated in the same session.
Clinics and patients give several reasons the crown often looks behind the front at the same month. Not in every case.
Blood Supply
Hair Palace, a clinic in Budapest, Hungary, says the crown has a slightly lower blood supply than the front of the scalp. It says this can make growth and healing slower.
One r/HairTransplants poster at five months asked whether it is normal "that crown takes longer." A commenter replied: "Crown has low blood flow and is slower than the front."
A photo can't tell you why a particular crown is slow. That question belongs with the surgeon who examines you.
Hair Growth Cycles
Hair Palace also says the crown may have a higher share of follicles in a resting or shedding phase at any one time. That can delay visible growth. It doesn't always.
The American Academy of Dermatology (AAD) gives a general timeline for all transplants, not the crown alone. It says "between two and eight weeks after the surgery, the transplanted hair will fall out. This is normal."
By the third month, the AAD says, "the hair may look thinner than before you had the transplant."
No reliable source publishes a month-by-month percentage of grafts grown, for the crown or anywhere else.
The Swirl and Hair Direction
The crown, also called the vertex, grows in a whorl, the spiral point where hair changes direction. Hair Palace says a crown transplant needs a careful approach to mimic that swirl.
The ISHRS says a natural look depends in part on recipient-site angling, distribution, hair curl direction and correct graft placement. So check the whorl first when you look at a crown photo.
One r/Hairtransplant poster 12 months after 2,500 crown grafts described incomplete coverage around the "whirl area and sides/edges of the crown."
Density and Coverage
Coverage means the scalp no longer shows through at a glance. Density is how much hair sits in each square centimeter. Which one is the photo showing?
Absolute Hair Clinic in Bangkok calls the crown a "density sink" in its plan for advanced hair loss. It says conservative planning is required there.
The donor area sets the limit. The ISHRS says hair removed from the donor area for FUE doesn't grow back there, and that donor use has to leave enough for future needs. So every graft used on the crown is one the front can't use.
Individual Healing and Growth Rates
Hair Palace says some patients may need up to 15 months to see full results in the crown.
In one r/HairTransplants thread, a poster said their crown felt as thick as the rest of their hair only after about 15 months. Others in the same thread reported 14 or more months, and 18 months or more.
A follow-up month on a photo tells you how much time has passed. It doesn't tell you how your own crown will look at that month.
Compared With the Front and Other Areas
In a six-month update after a hairline-plus-crown procedure, an r/HairTransplants poster wrote that the front started growing after two months. At six months, the crown was "just starting to fill."
That poster estimated about 1,700 of their 3,700 grafts went to the hairline and the rest to the crown.
A full front can also sit next to an untreated crown. At HairTran, one patient's doctor advised putting about 3,500 grafts into the front and midscalp instead of spreading them to the crown, then revisiting the crown in a year.
That patient later wrote that their crown hadn't been transplanted. Before you judge any crown photo, ask: was the crown treated at all?
What Patients Call It: "The Black Hole for Grafts"
In a 15-month crown thread, a commenter wrote: "The crown is a black hole for grafts." The original poster, who estimated about 700 grafts, replied that the crown was "definitely a difficult part to cover."
On HairLossTalk, a member with 1,000 crown grafts said they saw no significant difference. Another member said crown results can be the least noticeable.
Before-and-After Photos of the Crown
If your hair loss is in the crown area, a front-facing photo answers the wrong question. You need a top-down view of the scalp, from a case where the crown was treated.
Crown Before/After With Graft Counts
A graft number under a photo may belong to the whole session. A Bangkok Hair Clinic patient reported 2,725 grafts across hairline, midscalp and crown, and separately stated 825 for the crown.
Without that second figure, you couldn't tell how much of the total went to the crown. Our before-and-after guide covers lighting, angles and other photo conditions in more detail.
A Thai Clinic's Crown (Top) Gallery
Thailand Hair Transplant Center, a clinic in Chiang Mai, publishes a Top - Crown gallery with separate Top and Crown sections. The clinic says the images give a rough idea of outcomes and of how many grafts a case might need.
The gallery's text doesn't give a graft count, date or follow-up month for each case. So use it for a rough idea, as the clinic says, not to time your own crown.
The same clinic publishes a separate month-by-month crown sequence: before, week two, then months two through nine. The page doesn't state the crown graft count, the operation date or the patient, and doesn't say whether the views were taken under matched conditions.
How to Compare a Crown Before-and-After Result
Use the record below for every crown case you look at, from a clinic or a patient. It's a reading tool. It doesn't score a clinic.
The Six Details to Check
1. Source: the clinic page or patient post, with a link you can go back to. 2. Dates: the procedure date and follow-up month. A post date isn't a surgery date. 3. Treated areas: crown only, or crown shared with the hairline or midscalp. 4. Total grafts: record it, but don't work out the crown's share from it. 5. Crown allocation: only if the source states it separately. 6. A matched top-down view: before and after from above, at a similar angle, lighting and hair length.
Write "not published" wherever a source leaves a field empty, and never estimate it.
A case with blanks can still be useful, if you can see which fields are blank. The Bangkok Hair Clinic case above fills in crown allocation.
The Chiang Mai clinic's sequence fills in follow-up months but not allocation. Neither gives you everything.
What the Crown Looks Like Month by Month
The stages below come in a rough order. The months attached to them vary from person to person.
Right After the Procedure
Bangkok Hair Clinic says scabs may form within 24 hours and usually shed between days 4 and 14.
Photos from this stage show where on the scalp the grafts were placed. They tell you nothing yet about final density.
First 2 Months
The AAD says the transplanted hair falls out between two and eight weeks after surgery, and that this is normal. Bangkok Hair Clinic says some newly transplanted hair may shed around week four.
The HairTran patient from the opening wrote that nearly all of their transplanted hair had fallen out by about six weeks.
The ISHRS describes a separate side effect, temporary telogen effluvium: shedding of your original, native hairs.
4 Months
Bangkok Hair Clinic says new growth often becomes visible from months three to four, or weeks 10 to 16. The AAD adds that by month three, hair may look thinner than before the transplant.
A DHT Clinic patient of Dr Path posted on 5 January 2026 that they had passed the "ugly duckling stage" at three months. They described baby hairs that still needed to mature and thicken.
That was a 3,028-graft case placed in the front part of the head, so it shows how the front looked at three months, not the crown.
6 Months
HairTran's follow-up schedule says "noticeable growth begins" at six months. Thailand Hair Transplant Center says the crown in its month-by-month sequence looked denser at month six than the prior month.
The HairTran patient's crown at six months still had a notable bald spot, smaller and partly filled. So don't judge the crown's final result at six months.
Each of these is one case or one clinic's description. None of them predicts how your crown will look at six months. Our six-month guide covers what to raise at a six-month review.
8 Months
Thailand Hair Transplant Center says that by month eight in its crown sequence, just about everywhere on the bald area was covered. The page's crown sequence gives no graft count or patient details.
At nine months, a HairLossExperiences member reported bald areas on the crown. After the 12-month mark, the same member said the density was not really satisfactory to them. An administrator replied that crowns use a lot of grafts and may need another pass for more density.
12 Months
HairTran schedules a 12-month final assessment, with the doctor comparing before-and-after photos. The AAD says: "Most patients see results between six and nine months after surgery. For some patients, it takes 12 months."
15 to 18 Months
Surgeons generally ask you to wait 12 to 18 months before judging the final result. Some crowns settle sooner. That window is advice about when to judge, so treat it as a review date for your crown.
The poster of the 15-month crown thread, who estimated about 700 grafts, published a 15-month update. Later commenters in that thread reported very little crown growth at six months and a thin crown at 14 months.
Patient-Documented Timelines at 6, 12 and 18 Months
| Follow-up | Thai record | Other patient reports |
|---|---|---|
| 6 months | HairTran, 3,300 grafts including the crown, crown allocation not published. Bald spot smaller and partly filled (posted 21 December 2023) | A commenter at six months: "very little growth in the crown area" |
| 12 months | No Thai crown-only record with a graft count found | 2,500 crown grafts: incomplete coverage around the "whirl area and sides/edges of the crown" |
| 15 to 18 months | No Thai crown-only record with a graft count found | About 700 grafts: 15-month update. One poster: crown felt as thick as the rest of their hair after about 15 months |
We didn't find a Thai patient who posted a crown-only result at 12 or 18 months with a graft count. If you find one, run it through the six details above before comparing it with your own crown.
Realistic Density and Graft Counts for the Crown
This section is about reading the numbers that sit beside crown photos. Graft planning itself is on our crown guide.
Graft Counts by Crown Size
No reliable source maps crown size to a graft count. Thai clinics publish planning ranges instead.
Thailand Hair Transplant Center lists "Crown 1500 - 2500 Grafts" on its home page. Absolute Hair Clinic's advanced-loss plan lists an optional Stage 2, after 12 months, of an additional 1,500 to 2,500 grafts for crown pattern work and density enhancement of the frontal and mid-scalp zones.
Patient figures run wider. They include about 700 grafts at 15 months, 825 crown grafts in a Bangkok Hair Clinic case and 2,500 crown grafts in a 12-month case.
So which number fits your crown? That's a question for the surgeon who examines you.
Hair Caliber and Texture Change the Number
Caliber means the thickness of each strand. Thailand Hair Transplant Center says curly, thick-shaft hair can need fewer hairs for coverage.
It says straight, fine hair may need more hairs, and possibly extra sessions, for similar density. So two 1,500-graft crowns on different hair types may not look alike.
Realistic Density Targets by Zone
A follicular unit is a natural group of one to four hairs. Absolute Hair Clinic publishes these targets for advanced (Class VI-VII) loss:
| Zone | Absolute's target for Class VI-VII loss |
|---|---|
| Front | 35-45 follicular units per cm2 |
| Midscalp | 25-30 follicular units per cm2 |
| Crown | One listed option: light coverage, 15-20 follicular units per cm2 |
Bangkok Hair Clinic says it aims for 40 to 50 grafts per square centimeter "when appropriate." It says varying densities may be recommended in areas that have existing hairs, to maintain donor site integrity.
The Bangkok Hair Clinic patient reported 40 grafts per square centimeter in the crown and 45 in the front. These are plans and targets. A target isn't the density you're promised. It's a starting point.
When the Crown Needs a Second Session
Bangkok Hair Clinic says multiple sessions may be advised for advanced loss or limited donor hair. It spaces them at least 12 months apart, for recovery and assessment.
Whether your crown needs another pass is a question for an examination. Our second hair transplant guide covers timing and donor limits.
What Thai Clinics Publish About Crown Cases
We compare what clinics and their patients publish. We haven't visited these clinics.
Thai Patient-Documented Crown Timelines
| Clinic | Grafts | Crown allocation | Follow-up published | Matched top-down view | Source |
|---|---|---|---|---|---|
| Bangkok Hair Clinic, Dr Jate | 2,725 across hairline, midscalp and crown | 825, at 40 per cm2 | Two-month update | Not stated | r/HairTransplants, posted 22 July 2026 |
| HairTran, Dr Patty | 3,300, DHI/FUE, including the crown | Not published | Six months: bald spot smaller, partly filled | Not stated | r/HairTransplants, posted 21 December 2023 |
| HairTran | About 3,500 to front and midscalp | None, crown deferred | Six-month review; crown not transplanted | Not stated | r/HairTransplants, posted 2 July 2023 |
Post dates are when the patient wrote.
What a Thai Clinic's Crown Strategy Looks Like
Absolute Hair Clinic's plan for Class VI-VII loss lists deferring crown work until after medical stabilization as one crown option. It also lists an optional Stage 2, after 12 months, that includes crown pattern work.
The HairTran deferred-crown case shows the same order from the patient's side: front and midscalp first, with the crown to be revisited in a year.
Thailand Hair Transplant Center says some people choose another session even with a smaller bald area, depending on what they expect.
The 18-Month Anchor for a Staged Crown
If a clinic stages crown work after 12 months, that later session starts its own growth period. An update posted 18 months after the first surgery could be an early update on the crown.
So when a crown photo is labeled "18 months post-op," ask one question: 18 months since which procedure?
How Much a Crown Hair Transplant Costs in Thailand
The figures below are general per-graft and session prices, not crown packages. Our crown guide covers cost in more detail.
Per-Graft Prices
Patients report paying ฿36-123 per graft ($1.09-$3.71) across five Bangkok clinics (31 reports and quotes, 2023 to 2026).
Most reports at Absolute, Bangkok Hair Clinic and Hairsmith fall at ฿65-90 per graft ($1.96-$2.72). A few run outside that.
Total Cost by Graft Count
Hairsmith publishes ฿80 per graft ($2.41), labeled 2025 pricing, with a minimum of 1,000 grafts.
DHI Thailand publishes about ฿50,000-100,000 ($1,510-$3,020) for 1,000-2,000 grafts and about ฿100,000-200,000 ($3,020-$6,040) for 2,000-4,000 grafts (both clinics read 17 October 2026).
Patient-Reported Costs With Dates
The Bangkok Hair Clinic patient, in a July 2026 post, wrote: "Fee was 129,000THB for first 1500 grafts and additional 75THB per graft." That is ฿129,000 ($3,890) for the first 1,500, then ฿75 ($2.26) a graft.
The patient had 2,725 grafts across the whole head, not the 825 crown grafts alone. The patient put the total at about US$6,650 including PRP, in their own dollars.
A DHT Clinic patient who had 3,028 grafts by FUE with Dr Path recalled a price of ฿123 per graft ($3.71) in September 2025.
Dollars at ฿33.14 = $1 (European Central Bank reference rates, 22 September 2026).
What to Expect, and When to Judge the Result
Realistic Results
The ISHRS says a natural look depends in part on design, angling, distribution, curl direction and graft placement.
Ask your surgeon one thing first: what's the crown goal in your plan, light coverage or more density? Then ask to see a comparable crown case with the six details filled in.
Success Rate
No Thai clinic material we read publishes a success rate for crown transplants alone. A gallery can't supply one, because it shows the cases a clinic chose to publish with no count of the rest.
When the Crown Is "Done"
Agree on a review date with your surgeon, counted from the procedure that treated your crown. If the crown was deferred to a second session, the count starts there.
Is the crown still growing? A HairLossExperiences member asked that at nine months. Your before photos, taken from above, are the fairest thing to compare against.
If you have a crown plan from a Thai clinic, you can send it to us with your questions.
Frequently Asked Questions
Where can I see crown before-and-after photos instead of hairline photos?
Thailand Hair Transplant Center in Chiang Mai publishes a Top - Crown gallery and a separate month-by-month crown sequence. Patient posts on r/HairTransplants add dated cases.
For each one, check the follow-up month, the treated areas, whether the crown's own graft count is stated and whether there is a matched top-down view. Write "not published" for anything missing.
Is very little crown growth at six months normal?
The AAD says most patients see results between six and nine months, and some take 12. One HairTran patient still had a notable crown bald spot at six months, smaller and partly filled.
Hair Palace says some crowns need up to 15 months. If you're worried, ask your surgeon to compare your crown with your own before photos.
Does the crown take longer than the front?
Clinic explanations and patient accounts often say so. Usually, not always. Hair Palace points to a slightly lower blood supply in the crown, and says it may have more follicles in a resting phase.
One patient with a hairline-plus-crown case saw the front start at two months, while the crown was "just starting to fill" at six. It's a pattern people report, not a rule.
Should I expect more crown density after 12 months?
Some patients report more. Not all.
Where multiple sessions are advised, Bangkok Hair Clinic spaces them at least 12 months apart, and Absolute Hair Clinic's advanced-loss plan lists an optional Stage 2 after 12 months that includes crown pattern work. Your surgeon decides whether a second pass makes sense for your donor area.
Can the crown improve between 12 and 18 months?
Some posters say it did. One said their crown felt as thick as the rest of their hair only after about 15 months.
Another reported a thin crown at 14 months. Surgeons generally ask you to wait 12 to 18 months before judging the final result, so a 12-month crown photo may not be the last one worth taking.
